Advance Care Planning (G78720)

Purpose

This fee premium is intended to support discussions with capable patients about their beliefs, values, wishes or instructions for future health care to develop an advance care plan that will be used to make health care decisions should the patient become incapable.

Sizing it Up

An adult may have advance care planning discussions with close family or trusted friends and health care providers. When an adult's wishes or instructions for advance care planning are written down, they become an Advance Care Plan.

Things to Know

The fee is applicable for advance care planning for patients with a chronic medical illness or complex co-morbidities, and a deteriorating quality of life or end-stage disease state.
