Aim: To enhance the role of specialists in supporting primary care practitioners to increase their knowledge and confidence in early diagnosis, and culturally sensititive treatment and management of people with dementia and their families.
Summary: More timely diagnosis of dementia with prompt referral to community services and resources has the potential to increase the quality of life and care for people living with dementia and their families.
This overarching aim will be achieved through the development and implementation of activities in three key streams:
1. An integrated interprofessional collaborative practice guideline/pathway in three designated communities in FH where specialists and a Division of Family Practice (DoFP) are working in collaborative teams,
2. A competency based dementia education program focused on early diagnosis and referral to community supports that will be tested within Divisions of Family Practice (DoFP) within Fraser Health, and
3. A public awareness and education campaign targeted for the South Asia population.
