Specialists Team Care

Specialists Team Care

Purpose

Team care is an interprofessional, structured, and personalized program designed to assist patients in working with the right health care professionals who then coordinate their expertise to help the patient optimize their health.  High functioning teams have been linked to increases in efficiency, improved quality of care, positive patient outcomes and experiences and decreases in workloads and clinician burnout as well as turnover.

The Specialists Team Care Collaborative (STCC) strives to create a network of community specialists, which collaborate and innovate across and within specialties, and with other health professionals, to offer high quality and timely specialist care. 

Sizing It Up

Specialist-led teams will be supported to implement a team care model through interactive, multidisciplinary learning sessions and action periods that will empower the specialist and team to continue well beyond the life of the Collaborative.
